Job Description

The Behavioral Health Provider is a member of the multidisciplinary team who carries out support functions; assists in treatment, education, support and consultations with individuals, children, and families. He or she will interact as an agent of Sunset Community Health Center, Inc. (Sunset) with outside entities and serve as a Behavioral Health Consultant to Sunset medical providers and other team members. May be assigned to serve on special committees and conduct various meetings as necessary. Required to be familiar with, abide by, and enforce all State of Arizona, Behavioral Health Agency, and Regional Behavioral Health Authority (RBHA), Office of Behavioral Health Licensing (OBHL), and Sunset Community Health Center regulations.

  • Must be able to obtain a fingerprint clearance card. If not already obtained, Fingerprint Clearance card application must be submitted within 5 business days of employment.
  • Must be able to successfully pass a background check and a health and drug screen.

Education and/or Experience:

Requires a thorough working knowledge of the State of Arizona Behavioral Health Agency/Medical rules, regulations and standards with the ability to administer these rules, regulations and standards.
Must possess a valid State of Arizona driver’s license. Must have Master Degree in behavioral health related field (Licensed Clinical Social Worker, Counseling, Psychiatrist, Behavioral Health Medical Practitioner, or Psychologist). Must be independently licensed through the Arizona Board of Behavioral Health Examiners.
Experience in a primary care setting helpful. Must have at least 2 years of directly related experience in any other Behavioral Health care unit.
Must meet the requirements of A.A.C. Title 9, Chapter 20.
First Aid certification

LCSW/LPC preferred, LMSW/LAC considered
Current Pro-Assault Crisis Training (ACT) Certification
Bilingual in English & Spanish preferred.
